This module will solidify the carer identification and the understanding of support services by the General Practitioner following through with a referral.
Referral Process
If you would like to refer a carer to Carer Gateway please complete our Online Referral Form or alternatively call 1800 422 737 Monday to Friday between 8.00am and 5.00pm WA local time. An after-hours service is available outside of these hours, including weekends.
Alternatively, anyone can request a call back by completing an online form here.
Please Note: Services are free to access for anyone looking after a family member or friend with a disability, chronic condition, mental health challenge, an alcohol or substance dependency, or someone who is frail due to age. Access does not affect any other state or territory carer services, young carer bursaries or other services delivery though My Aged Care or the National Disability Insurance Scheme.
What Can a Carer Expect after your Referral –
The Carer Gateway team will contact the carer and book a planning session which can take up to 60 minutes and is conducted over the phone. This planning session will be done using a Carers Star Planning Process.
Mobile planning or face-to-face sessions is available as an alternative to over the phone planning sessions if the carers would prefer (face-to-face is only done in metro areas). These sessions can take place with the carer in their homes or community venue e.g. library, coffee shop, park etc.
These planning sessions will help identify the carer’s goals and ensure that an appropriate support plan is prepared, implemented, and evaluated.
From this planning session we will create an action plan of services and supports tailored to the carer’s individual needs.
